How to Place an Order

Doing business with us is easy. We take credit card orders (VISA, MasterCard, American Express) over the phone (or by FAX), and we take personal checks in the mail. Our phone number is easy to remember — 650-321-CERI. Our Mailing address is CERI, Post Office Box 4029, Menlo Park, California (abreviated “CA”), postal code “94026-4029” in the United States of America (“USA”).

How to Print an Order Form:

After clicking on the order form below, select the FILE-PRINT option of your web browser program to print the selected order form to your computer’s printer. Important: If the printing is too big and runs off the right side of the paper, use the TEXT-SIZE option on your web browser program to select the next-smaller font, and then print it again.

Ordering By Phone:

Call 650-321-2374 with your VISA, MasterCard, or American Express card ready.

Credit-Card Ordering By Fax:

Print the order form on your computer’s printer, then fill it out with all indicated credit card information (please include phone numbers in case we have a problem reading anything). The minimum credit card order is $5. Fax the order form to us at 650-323-3864. American Express credit card orders must also include the billing address for the account (if it is different from mailing/shipping address for the order). VISA and MasterCard orders do not require a billing address.

Ordering with Personal Check by Mail:
